We understand that not everyone can bring their own pet to appointments. If you or your spouse can not make it to your pets appointment please fill out a Medical Power of Attorney form and return it to our hospital. This will give your family member or friend permission to approve medical treatments and medications (not including euthanasia’s or new clients). Don’t forget to keep one on file for your pet sitter or boarding facility.
